While George is feeling a little less masculine because of the way Izzie and Meredith have been treating him (like a sister), Izzie is assigned to a patient about to lose all of his masculinity. Cristina, true to her competitive spirit, shows up early and assigns herself to an ill woman, who happens to have been Meredith's mom's scrub nurse. Alex, meanwhile, is busying himself copying and pasting pictures of Izzie's racy magazine spread all over the interns' locker room. And while Meredith is trying to cope with her mother's Alzheimer's, Derek treats a patient who has managed to get nails stuck in his head.

Inoltre, ricordandosi le sue prime esperienze, nota il carattere competitivo di Cristina e la mette più in volte in imbarazzo davanti al Dr. Burke. Nel frattempo, Meredith si occupa di un caso asportabile di tumore al cervello, ma purtroppo situato nella zona della memoria. Lei e Derek, che intanto le ha chiesto di mantenere per ora un rapporto solo professionale, cercano di trovare una cura alternativa. Tuttavia il paziente decide di operarsi lo stesso, ma Meredith cerca di convincere sua moglie a non autorizzare quella operazione perché altrimenti si sentirà male quando si avvicinerà a suo marito e lui non la riconoscerà. Intanto Meredith viene a conoscenza che l'infermiera di sua madre è in ospedale e quando le fa visita si confida con lei riguardo lo stato di salute di sua madre. Mentre Izzie ha problemi con il suo passato, Cristina non riesce a capire perché Burke non opera la signora malata di cancro e lui le dice semplicemente che vuole morire nel posto a lei più caro e che non hanno l'autorizzazione ad intervenire. Terminata la giornata, Meredith viene invitata da Derek per un caffè e la ragazza, cosciente della loro ultima conversazione, decide di accettare ugualmente il suo invito.
