毕业前夕，以逆转心衰为己任的王扬鸣因与导师在医学实验上发生分歧，毅然决定从科研转向临床，并顺利入职了全国心血管排名第一的瑞仁医院急诊科。忙碌的急诊科生活激起了王扬鸣的竞争本能，他冒险处理老人室颤，解救热射病农民工，救治脑中风病人，不断精进医术，并与胖子路易、祖闻达和刘非组成了患难与共的“急诊四杰”。四人相互见证了彼此的成长和改变，收获了友情爱情，也经历了聚散离合，但不变的是：哪里有最危重的病人，他们就会出现在哪里。当王扬鸣面对医疗生涯中最棘手的心脏病人，四杰重新相聚，同战困难，终获成功 。
