Following the events of ''Ring'', the body of List of Ring characters#Ryuji Takayama, former husband of Reiko Asakawa and father of List of Ring characters#Yoichi Asakawa, is examined by his friend and rival, Forensic pathology Mitsuo Andō. After he finds a cryptic note in Takayama's stomach, Reiko and Yōichi also turn up dead. Andō soon learns of a mysterious cursed videotape, haunted by the spirit of a murdered young woman. Rumor has it that anyone who watches the video will die exactly one week later. Despondent over the death of his own child, and believing that he is being guided by his rival's ghost, Andō decides to see the video for himself. After watching the tape, strange things begin to happen around him, and he soon discovers that the tape's restless spirit has different plans in store for him.
After the body of Sadako Yamamura is retrieved from a water well, her uncle Takaishi is summoned by police to identify her. Detective Omuta explains to Takaishi that forensics concluded Sadako may have survived in the well for thirty years. Forensics experts reconstruct her body, giving it to Takaishi, who gives his niece a burial at sea, hoping to be free from the guilt he has carried since her mother Shizuko committed suicide because of his actions. The police search for Reiko Asakawa following the sudden death of her ex-husband Ryuji Takayama and her father Koichi a week later. Mai Takano, Ryuji's university assistant, investigates his death, visiting Reiko's news office where her colleague Okazaki joins Mai in her search for answers. They find a burnt out videotape in Reiko's apartment, Mai sensing Reiko's father died the same way as Ryuji.
Thirty years before the events of ''Ring (film)'', Akiko Miyaji, a reporter whose fiancé and fellow reporter was killed during Shizuko Yamamura's publicized ESP demonstration years before, interviews Sudo, an elementary school principal and a former teacher of Shizuko's daughter, Sadako, attempting to gain information and evidence that Sadako inherited her mother's nensha powers. Meanwhile, a 19-year-old Sadako joins an acting troupe as an understudy as a therapy to her nightmares suggested by her doctor. Sadako has natural beauty and charisma for the play, infuriating her senior, Aiko Hazuki, whose relationship with the troupe director, Yusaku Shigemori, sours due to the latter's newfound favor for the young trainee. Aiko is later found murdered by a figure in white, thus Sadako takes her place for the upcoming play as the lead character. Sadako attracts and reciprocates the attention of the troupe sound director, Hiroshi Toyama, much to the disappointment of costume designer and Toyama's girlfriend, Etsuko Tachihara. While praised by Shigemori and Toyama, other troupe members grow to distrust and fear Sadako, as they suspect that she is the one who caused Aiko's death and other supernatural occurrences, including strange dreams pertaining to a well and an apparition of a girl in white with long hair very similar to Sadako.
A mysterious, white-clad man drops a long-haired woman into a well. The well is full of women, all with long hair, all dressed in night-dresses.
Takanori Andō (Kōji Seto) is waiting while Akane Ayukawa is giving birth to their daughter. Seiji Kashiwada's landlord sitting in a garden reads Kashiwada's letters and mutters that the girl is born. Another woman is accessing her laptop in her room when her husband calls her; she is suddenly possessed by a force from her laptop who forces her to commit suicide by stabbing her eye with a knife.
A social worker visits the residence of an elderly woman, only to find her strangled by an electric cord. A nearby video player suddenly turns on and begins playing the infamous cursed videotape. As the social worker watches, Sadako Yamamura manifests behind her. The social worker is eventually killed with a knife slitting her throat.
心理カウンセラーである茉優(池田エライザ)の勤め先に、警察に保護された少女が入院してくる。記憶を失い名前も言えない彼女のカウンセリングにあたる茉優だが、周囲で奇怪な現象が頻発する。同じころ、茉優の弟で動画クリエイターの和真(清水尋也)は、アップロードした映像の再生回数が伸びないことに焦りを感じていた。そこで、死者5人を出した火災の起きた団地に侵入し、心霊動画を撮ろうとする。
